1. Equipment Description

Owner Agrees To Rent To Renter The Following Pulsed Electromagnetic Field (PEMF) Equipment:

The Equipment Must Be Returned To Owner No Later Than 5:00 PM On The End Date Unless An Extension Is Agreed Upon In Writing.

Renter Acknowledges That They Have Inspected The Equipment And That It Is In Good Working Order. Renter Agrees To Return The Equipment In The Same Condition As Received, Normal Wear And Tear Excepted.

5. Use & Restrictions

Non-transferable: Renter Shall Not Sublet, Loan, Or Assign The Equipment To Any Third Party.

Proper Use: Renter Agrees To Use The Equipment Only As Directed In The Provided User Manual.

Contraindications: Renter Confirms They Do Not Have A Pacemaker, Internal Defibrillator, Cochlear Implant, Or Other Active Electronic Implant, And Is Not Currently Pregnant.

6. Loss, Theft Or Damage

Renter Assumes All Risk Of Loss Or Damage To The Equipment From Any Cause. In The Event The Equipment Is Lost, Stolen, Or Damaged Beyond Repair, Renter Agrees To Pay The Owner The Full Replacement Value Of $.

7. Limitation Of Liability & Release

Renter Acknowledges That Florida PEMF Is A Rental Provider And Not A Medical Provider. PEMF Therapy Is Not Intended To Diagnose, Treat, Cure, Or Prevent Any Disease.

  • Renter Hereby Releases Florida PEMF From Any And All Liability For Injuries Or Damages Arising Out Of The Use Or Inability To Use The Equipment.
  • Renter Assumes All Responsibility For Consulting With A Physician Prior To Use.

8. Governing Law

This Agreement Shall Be Governed By The Laws Of The State Of Florida. Any Legal Actions Shall Be Brought In The Courts Of Pinellas County, FL.

Identification Requirement: Renter Agrees To Provide A Clear, Legible Copy Of A Valid, Government-issued Driver's License (or State ID) At The Time Of Signing. This Identification Is Used Solely For The Purposes Of Identity Verification And To Protect The Owner's Equipment From Theft Or Unauthorized Transfer.

Privacy Policy & Data Security: Florida PEMF (The "Owner") Is Committed To Protecting The Renter's Personal Information.

  • Collection: Personal Data, Including Name, Contact Information, And A Copy Of The Renter's Identification, Is Collected For The Sole Purpose Of Executing This Rental Agreement.
  • Storage & Security: Owner Maintains Reasonable Administrative, Technical, And Physical Safeguards Designed To Protect The Personal Information Provided By The Renter Against Accidental, Unlawful, Or Unauthorized Destruction, Loss, Alteration, Access, Disclosure, Or Use.
  • Third-Party Disclosure: Owner Will Not Sell, Rent, Or Lease Renter's Personal Information To Third Parties. Data May Only Be Shared With Law Enforcement Or Collections Agencies In The Event Of Equipment Theft, Non-Payment, Or As Required By Florida Law.
  • Retention: Copies Of Identification Will Be Securely Destroyed Or Deleted Within [E.G., 30 Or 60] Days Of The Successful Return And Inspection Of The Equipment.
